– Help patients comply with their prescription medications.

– Better understand how the medications interact with each other and with other OTC medications.

– Determine if there are any actual or potential drug related problems not readily apparent

Care plan $225.00/1 hour

The plan may include interventions such as dose modification, dosage form

modification, discontinuation of drug, addition of drug, non-compliance

resolution, patient counselling and education.

• Development of the care plan is followed by:

– A private consultation with the patient.

– The definition of therapeutic monitoring goals.

– The execution of a Follow-Up Plan

Education services $90.00-$180.00/30 min

Disease and medication therapy management $30.00/10 min

Examples include (but are not limited to) specifically tailored programs for managing patients with asthma, diabetes, hypertension, or for patients who are taking anticoagulants

Smoking cessation $180.00/4 consultations

Vaccination administration $30.00

Other specialized/advisory consultation $225.00/hour
